Please fill out the application with all required information.  We will NOT call or email for anything other than verification or purchase information. 

YOU MUST attach a photograph of your First Responder ID with a clear expiration date for your certification.  POST ID etc.  *Retirement Cards are requested to provide the retirement year in the custom request section*

YOU MUST attach a photograph of your current badge.  We may need to custom create your engraving.

WE DO NOT retain photographs of badges or IDs at anytime for your privacy.  They are purely for the setup of your Slim Shield.
